  1. This form is for PURCHASE ORDERS only.  If you do not have a PO then arrangements can be made with HSTI - hsti@hssd.net.
  2. Fill in this form completely, then Review your selections carefully before submitting to verify your registration selections.  Please check all spelling and workshop selections thoroughly.
  3. Once your registration is correct, use the SUBMIT button to process your registration and CONFIRM on the next page to ensure that your registration will be processed.
  4. All information is required.
  5. INCOMPLETE registration forms will NOT be processed.
  6. For groups using the same PO number - you must fill in all information for each individual then one invoice will be sent to the appropriate address for each different PO # used. NOTICE: You MUST confirm this registration page after you submit.

You may need to view current availability by clicking the Pre-conference matrix link.  Registration will be on a first-come first-serve basis. If the workshop you desire doesn't appear then it is FULL.

Cancellation Policy:
Cancellations must be made and received in writing by June 1 for Conference and by May 1 for pre-conference workshops. (email to: hsti@hssd.net or fax 501-620-7829).  Refunds will not be made after this date. After SUBMITTING you must CONFIRM your selections on the next page.
